The lists of Assistants (1, 2, 3, etc) are automatically created. They contain the physicians of the index hospital that have declared that they participate in open procedures. 

To participate in open procedures, open My Account and in the Hospitals - Modules - Logbook session check the Participant in Open Procedures field.
